Authorizing the City Manager to execute a professional services agreement with Dr. David H. Cruz, M.D., for continued physician oversight of the City's Employee Health & Wellness Clinic. This agreement consolidates medical direction, workers' compensation review, and prescriptive authority. The initial term of the agreement is two years, with up to two additional one-year extensions upon satisfactory performance. Annual base compensation shall be $75,000 per year, with a 3% annual cost-of-living adjustment (COLA), not to exceed a total contract amount of $315,000.00. Funding is available in the Risk Management Fund - Employee Health and Wellness.
