Authorizing the City Manager to execute a professional services agreement with Dr. David H. Cruz, M.D., for continued physician oversight of the City’s Employee Health & Wellness Clinic. This agreement consolidates medical direction, workers’ compensation review, and prescriptive authority. The initial term of the agreement is two years, with up to two additional one-year extensions upon satisfactory performance. Annual base compensation shall be $75,000 per year, with a 3% annual cost-of-living adjustment (COLA), not to exceed a total contract amount of $315,000.00. BACKGROUND
The Physician agrees to render all physician services necessary within the scope of practice for occupational and preventive health services for its employees, retirees, and eligible dependents. The Physician shall provide professional medical oversight, worker’s compensation medical review, and prescriptive authority supervision in accordance with applicable Texas Law and the agreement.
